Required Medical Forms

  • Physical Form- required for kindergarten, 6th grade, and new students with updated immunizations.
  • Vision Form- Required for kindergarten students and new students from out of the country.
  • Dental Form- required for Kindergarten, 2nd grade, 6th grade students, and new students from out of the country.

COVID-19 Vaccine Information

​THURSDAYS - MODERNA VACCINATION CLINIC - 18+ YEARS OLD - 5127 OAKTON ST.
*Check back often as additional first dose vaccination appointments become available through the Health and Human Services Department. Vaccination appointments can be secured at local pharmacies and other providers. Search for area vaccination appointments. Vaccination appointments for individuals age 12+, including walk-in appointments, also are available at Erie Family Health Centers at 1285 Hartrey Avenue, Evanston.

In order to receive services, Children must live in Skokie, and are of the ages of 18 and under.

Immunizations:
​Immunizations by appointment only.  A complete record of your child's previous vaccinations must be provided prior to finalizing an appointment. 

Children who qualify for the immunizations program through the Skokie Health Department are:
  • Children ages of 18 and under
  • Children without healthcare insurance (uninsured)
  • Children who are covered by health insurance, but the insurance does not cover the cost of vaccines (underinsured)
  • Children who are eligible for Medicaid or enrolled in a Medicaid eligible program
  • Children who are of American Indian or Alaska Native decent
